1. THE SUBJECT OF THE AGREEMENT

1.1. Insurer undertakes to provide insurance coverage for persons traveling abroad, provided that the Policyholder paid the insurance premium in the amount and terms specified in the Contract insurance .

2. DEFINITION

An insurer is a legal entity that accepts in accordance with the insurance contract, the obligation to pay the sum insured - JSC "Kafolat Hayot Insurance " .

Policyholder - a legal or natural person who concludes an insurance contract in his own favor or in favor of another faces.

insured – a natural person indicated in the insurance policy in whose favor the insurance contract has been concluded and who has an insurable interest.

An insurance policy is a document of a certain form, which includes the terms of an insurance contract and confirms its conclusion.

An insured event is an event causally related to an insured risk, in the event of which the payment of insurance indemnity is provided in accordance with the insurance contract.

Insurance indemnity - the payment of a sum of money or the provision of services upon the occurrence of an insured event in accordance with the insurance contract.

Insurance premium – payment for insurance specified in the insurance contract.

Insurance period – the period of time for which the insurance contract is concluded.

Insurance program - a description of the totality of insured risks, which indicates for what losses, damages or damages and in what amount the Insurer pays insurance indemnity.

Sum insured - the amount of money for each insurance program, which is the maximum insurance indemnity payable in accordance with the insurance contract. The sum insured specified in the insurance contract applies to each Insured, unless otherwise specified in the insurance contract.

Sums insured for each individual type of insurance indemnity – sums insured for specific types of insurance indemnity relating to each Insured.

Travel – trip of the Insured outside the country of his residence. In the meaning of these rules, the journey begins when the Insured enters the territory specified in the insurance contract, crossing the border, and ends when the Insured leaves this territory, crossing its border on the way back.

Country of residence - the country of which the Insured is a citizen, the country of his permanent residence and the country that issued him a permanent or temporary residence permit.

Deductible – part of the loss or costs, expressed as a percentage or in monetary terms, which is not reimbursed by the Insurer. The deductible specified in the insurance contract applies to each Insured, each insured event and each insurance contract.

Accident - a sudden incident, independent of the will of the Insured, caused by the impact of external forces, and as a result of which the health or life of the Insured was harmed. Surgical intervention, the consequences of surgical intervention, abortion and childbirth are not equated to an accident.

Costs are direct losses incurred by a person. In the meaning of these rules, losses do not include lost profits and moral damage.

Beneficiary – a person specified in writing by the Insured, to whom, in the event of the death of the Insured, the insurance indemnity is paid. If the Insured has not indicated the Beneficiary in writing, in the event of the death of the Insured, the Insurer will pay insurance compensation to the heirs of the Insured in accordance with the current regulations of the Republic of Uzbekistan.

Medical evacuation - from a medical point of view, transportation of the Insured to the country of residence, necessary and permitted by the doctor. In the event that evacuation is carried out to the place of permanent residence of the Insured, which is not Uzbekistan, the Insurer pays insurance compensation only in the amount that would have been spent on evacuation to Uzbekistan.

Repatriation - transportation of the remains of the Insured to the airport of the country of residence, if the transportation was carried out by plane, or to the mortuary, if the transportation was carried out by another mode of transport. At the direction of relatives, repatriation can be carried out to a country of residence, which is not Uzbekistan, provided that the Insurer pays insurance compensation in the amount that would be spent on repatriation to Uzbekistan.

Severe sudden illness - an unforeseen illness that did not manifest itself before the start of the trip, which requires emergency care.

Emergency care - the minimum possible medical care, in case of non-provision of which the life of the Insured is endangered.

Chronic disease is a disease which continues for a long time and periodically repeats.

An exacerbation of a chronic disease is a manifestation of the characteristic symptoms of a chronic disease, which require emergency care.

Assistance Service - a specialized organization that provides round-the-clock advisory assistance to the Insured Person throughout the territory of the Policy, providing the Insured Person with assistance in organizing and providing medical, medical , transport and other services upon the occurrence of an insured event and having the right to control the course of treatment.

3 . OBJECT OF INSURANCE

3.1 . The object of insurance is the health and life of the Insured.

3.2. In accordance with the terms of the contract, when insuring the health of a person, the principle of compensation is applied, which compensates for the costs of medical and transport services during the trip, if these services are provided to the Insured Person due to a sudden illness or accident.

3.3. When insuring the life of a person, the principle of compensation is applied, in which the payments for repatriation services are compensated if the death of the Insured has occurred as a result of an accident or a sudden serious illness.

4. INSURANCE RISKS

4.1. Insurance risks are:

4.1.1. sudden illness of the Insured Person, which requires emergency assistance;

4.1.2. an accident in which the Insured Person needs emergency assistance.

5 . EXCEPTIONS

5.1. The insurer does not reimburse costs:

5.1.1. If the purpose of the trip of the Insured was to receive medical assistance or advice;

5.1.2. If the Insured knew before his trip that he would need medical assistance during the trip;

5.1.3. If the presence of alcohol, narcotic or psychotropic substances in the body of the Insured during the first visit to the doctor, which is associated with the onset of the insured risk, or the use of medicines not prescribed by the doctor;

5.1.4 . If the use of alcohol, narcotic or psychotropic substances or the use of medicines not prescribed by a doctor has resulted in the occurrence of an insured risk;

5.1.5. If, upon the occurrence of an accident, the presence of alcohol, narcotic or psychotropic substances in the body of the Insured or the use of medicines not prescribed by a doctor was detected;

5.1.6. In the field of psychiatry, incl. tantrums, sudden stress reactions;

5.1.7. If they are associated with epilepsy or other diseases of the nervous system;

5.1.8. If they are related to family planning or infertility treatment, as well as preventive examinations of pregnant women, abortions;

5.1.9. If they are related to the treatment or saving the life of a person not specified in the policy;

5.1.10. If they are associated with venereal, sexually transmitted diseases, AIDS and all diseases caused by HIV infection;

5.1.11. If they are associated with burns when exposed to solar ultra - violet rays;

5.1.12. If they are associated with diseases for the prevention of which vaccination is mandatory or recommended;

5.1.13. If they are associated with allergies;

5.1.14. If they are associated with cancer;

5.1.15. If they are associated with operations that are not urgent;

5.1.16. If they are associated with rehabilitation activities;

5.1.17. If they are associated with vaccination or other types of prophylaxis, as well as health disorders caused by vaccination or other types of prophylaxis;

5.1.18. If they are associated with laser vision correction;

5.1.19. If they are associated with treatment with non-traditional methods of medicine, without specifying a diagnosis or treatment that does not correspond to the established diagnosis;

5.1.20. If they are associated with operations on the heart or blood vessels, tissue or organ transplantation, prosthetics;

5.1.21. If they are associated with the treatment of tuberculosis and its varieties;

5.1.22. If they are associated with diabetes.

5.1.23. If they are related to the purchase or repair of assistive devices (for example, the purchase of glasses, prostheses, crutches);

5.1.24. If they are associated with the acquisition of vitamins, nutritional supplements, food supplements, herbal products, homeopathic remedies;

5.1.25. If they are associated with services to provide increased service or comfort.

5.2. The insurer shall not reimburse the costs if the insured risk occurred with the Insured:

5.2.1. when attempting or committing suicide;

5.2.2. when riding a motorcycle, scooter, quad bike or jet ski;

5.2.3. when riding on rollers;

5.2.4. when flying on an aircraft that is not used by the aviation community , is not registered as a passenger vehicle on a specific route, as well as on a glider, hang glider, parachute, paragliding, etc .;

5.2.5. in the performance of official duties in any military structure;

5.2.6. when practicing any sports at a professional or amateur level, such as, but not limited to, alpine skiing, mountaineering, trekking, winter sports, ski jumping, bungee jumping, snowboarding, any kind of auto and motor sports, air types of sports, combat sports, diving (snorkeling, diving) speleology, parachuting, rafting, surfing, if the insurance contract does not indicate “SPORT” in the “Special Tariff” column;

5.2.7. when performing physical work, if the insurance contract does not indicate “WORK” in the column “Special tariff”.

5.2.8 . when skiing or snowboarding outside special tracks designed for this purpose and specially processed and equipped, even if the insurance contract specifies the risk associated with skiing, snowboarding, professional or non-professional sports;

5.2.9. when diving or diving at a depth of more than 30 (thirty) meters or doing these sports in the Arctic Ocean and adjacent seas, even if the terms of the insurance contract include coverage of diving;

5.3. If the insured risk occurred to the Insured at the time when he was in the territory of the Russian Federation, the Insurer does not pay the costs incurred by the Insured when visiting private clinics and centers, as well as American, German, French medical centers and clinics, and joint US - Russian, etc. medical institutions.

5.4. The insurer does not reimburse the costs of care and accommodation of the patient, as well as the storage of the body of the deceased.

6 . GENERAL EXCLUSIONS

6.1. The insurer shall not indemnify for losses that are directly or indirectly caused by:

6.1.1. Social disturbances (strikes, civil disobedience, etc.), military actions (with or without a declaration of war), civil wars, direct or indirect effects of nuclear explosions, nuclear energy or radioactive substances, decisions of administrative and law enforcement institutions, terrorist acts, those. acts that are disseminated as force or the use of force, or the threat of their use, by any person or group of persons who act alone or with the help of any organization or on behalf of it, which is done on political, religious, ideological or ethnic reasons, in order to influence the government or to keep the society or any part of it under fear. Any losses or expenses that are directly or indirectly related to the prevention of terrorist acts are not paid.

6.1.2. Global catastrophes or natural disasters, pandemics, epidemics, incl. influenza pandemic and/or epidemic;

6.1.3. Malicious intent of the Insured or due to fault, which, in terms of its degree in relation to the payment of indemnity in civil law consequences, is equated to malicious intent;

6.1.4. Voluntarily exposed to additional risk, excluding the case of saving a person's life;

6.1.5. Participation of the Insured in any type of competition related to speed.

6.2. The Insurer shall not reimburse the costs if the Insured or the Policyholder:

6.2.1. Provides the Insurer with incorrect, misleading or incomplete information or fails to inform the Insurer of a change in circumstances regarding the object of insurance, the insured risk or conditions insurance contracts;

6.2.2. Does not provide the Insurer with the necessary documents that confirm the occurrence of the insured event and the amount of losses caused as a result of the insured event;

6.3. In accordance with these rules, the Insurer does not indemnify:

6.3.1. Indirect losses, lost profits, moral damage;

6.3.2. Costs in connection with an accident at work and costs in connection with occupational diseases for the Insured, who work as miners, members of the crew of aircraft or ships, with nuclear reactors, toxic chemicals, decompression chambers, with explosive substances, perform stevedoring work, serve in the military or work offshore, such as on an offshore oil platform.

6.3.3. Compensation of the Insured as an employee.

7. SUM INSURED, INSURANCE PREMIUM AND PAYMENT PROCEDURE

7.1. The sum insured is specified in the Insurance Policy.

7.2. The total sum insured under this contract is specified in the insurance policy in US dollar / Euro.

7.3. The total insurance premium under this contract is specified in the insurance policy in the US dollar / Euro. The insurance premium is transferred to the settlement account of the Insurer within 3 banking days from the date of signing this agreement.

7.4. The insurance policy is issued to the Policyholder after the receipt of the insurance premium on the Insurer's current account.

8. TERM OF INSURANCE CONTRACT

8.1. The period of insurance is set according to the period of stay of the Insured person outside the Republic of Uzbekistan, but not more than one year and is indicated in the Policy.

8.2. Insurance comes into force at 00:00 on the day specified in the Policy as the date of commencement of the insurance period, but not earlier than the moment the Insured person crosses the border of the insured territory (marked by border guards in the passport), and terminates at 24:00 on the day specified in the Policy as the the end of the insurance period, but no later than when the Insured person crosses the border of the insured territory (marked by border guards in the passport).

8.3. The term of insurance is fixed in the insurance policy.

9 . OBLIGATIONS OF THE INSURED / INSURED PERSON.

9.1. The insurer is obliged:

9.1.1. - issue an insurance policy to the Insured/Insured after the receipt of the insurance premium on the Insurer's settlement account.

9.1.2. - in the event of an insured event, pay insurance compensation in accordance with the insurance contract.

9.2. The Policyholder/Insured is obliged to:

9.2.1. - during the term of the insurance contract, immediately notify the Insurer of all changes in the submitted documents, as well as other circumstances that may increase the risk of an insured event or the amount of possible damage in the event of an insured event;

9.1.2. immediately, as soon as possible, inform the Insurer about the occurrence of the insured risk, submit a written application for the insured risk and all documents related to the insured event;

9.1.3. upon the occurrence of an insured event, immediately, as soon as possible, notify the Insurer or the partner company of the Insurer about it, about which the Insured is informed when concluding the insurance contract in order to receive instructions on further actions.

10. RESPONSIBILITIES OF THE PARTIES:

10.1. The Insurer undertakes to provide the Insured with insurance coverage for persons traveling abroad.

10.2. The Policyholder undertakes to familiarize the Insured Persons with the rules of insurance when traveling abroad .

11. INSURANCE INdemnity

11.1. In accordance with these rules, the insurer shall indemnify the following costs:

11.1.1. expenses for emergency assistance, which was provided to the Insured Person during the trip in connection with the onset of the insured risks referred to in clause 4 .

The following payments are included in the cost of emergency care:

- for inpatient treatment;

- for outpatient treatment;

- for the purchase of medicines under the written prescription of the attending physician;

- for dental treatment, limited only to pain relief and temporary measures (X-ray, medication, temporary filling or tooth extraction) in the amount of up to 150 USD (one hundred and fifty US dollars / Euro);

- expenses for the provision of first emergency medical care during pregnancy for a period of not more than 30 weeks, not exceeding the limit of 500 USD (five hundred US dollars / Euro);

The Insurer pays medical expenses until the moment when it becomes possible to transport the Insured to the place of residence, but in any case not longer than 30 (thirty) days from the date of expiration of the insurance contract, and in case of insurance coverage for multiple trips within one year - no longer than 58 (fifty eight) days from the date of commencement of the journey.

11.1.2. Transportation costs.

These costs include the following payments:

- for transportation to a medical institution, provided that this institution provides emergency care in case of a sudden illness or accident;

- for medical evacuation, i.e. transportation of the injured Insured to a hospital in Uzbekistan;

- for transportation of the accompanying Insured Person only during medical evacuation, if the accompaniment is necessary at the direction of the attending physician;

11.1.3. for repatriation;

11.1.4. expenses in connection with search and rescue activities in the event of the occurrence of the risk referred to in clause 4.1.2.;

11.1.5. expenses associated with burial in the host country (excluding funeral services);

11.1.6. expenses for telephone conversations in order to determine the Insured to a hospital during the trip, but not exceeding 20 USD (twenty US dollars / Euro).

11.2. When indemnifying the costs of medical evacuation or repatriation, if before the commencement of the provision of transport services the Insured or his authorized person did not agree in writing with the Insurer on the procedure and amount of costs for the provision of these services, the Insurer shall reimburse the costs for these services within the minimum amount for which these services could be provided by the Insurer.

11.3. The insurer reimburses medical expenses for surgical operations only on the condition that this manipulation is urgent and cannot be carried out after medical evacuation to the country of residence, i.e. only in cases where the life of the Insured may be in danger if the operation is not performed immediately.

11.4. When reimbursement specified in clause 6.1. of these cost rules, a deductible of 50. USD (fifty US dollars / Euro) is applied for each insured event.

11.5. The Insurer reimburses the costs for medical services provided in medical centers, clinics and private doctors in Turkey, Egypt, Greece and Bulgaria, with whom the Insurer has not concluded cooperation agreements and whose telephone numbers are not specified in the insurance contract, only in the amount whatever they were reimbursed by the Insurer, paying for the same services to its partners, whose phone numbers are indicated in the insurance contract.

11.6. Decisions on the duration of treatment, the need for operations and the implementation of medical evacuation are made by the Insurer, its Assistance Service and the attending physician of the Insured. If the Insured solely decides to perform an operation, diagnostics or treatment outside the country of residence, the Insurer has the right not to pay the insurance indemnity.

1 2 . PAYMENT PROCEDURE INSURANCE COMPENSATION .

12.1. In order to receive the insurance indemnity, the Insured is obliged to fulfill the conditions mentioned in these rules, as well as to provide the Insurer with:

12.1.1. application in the prescribed form;

12.1.2. confirmation of the conclusion of the insurance contract (insurance policy or its copy);

12.1.3. original documents of those instances that confirm the occurrence of the insured event and the amount of damage;

12.1.4. additional documents requested by the Insurer about the insured event.

If the Insured does not fulfill or does not fully fulfill the obligations referred to in this paragraph, the Insurer has the right to refuse to pay the insurance indemnity to the Insured or reduce it.

12.2. In the event of the death of the Insured, the Insurer must submit a copy of his death certificate, presenting the original, persons claiming insurance compensation must present a copy of the identity document and its original; if the Insured did not indicate the beneficiary under the contract, then the heirs must present documents confirming the right to inherit.

12.3. In order to receive compensation for medical expenses, it is necessary to provide documents confirming the diagnosis, which were issued to the Insured by a certified doctor or medical institution.

12.4. The insurer, within 15 days after considering the submitted documents on the occurrence of the insured risk, makes a decision on the payment of insurance compensation, its reduction or refusal. The Insurer shall, within 10 (ten) working days from the day when the decision was made, notify the Policyholder/Insured about the refusal to pay out.

12.5. The insurer shall pay out the insurance indemnity within 15 days from the moment when the decision on the payment of the corresponding insurance indemnity was made.

12.6. The terms of the insurance contract (policy) provide for the payment of insurance indemnity for each type of insurance indemnity in the amount provided for in Table No. 1, which is an appendix to these rules.

12.7. Upon the occurrence of an insured event, the Insured has the right to apply for assistance to the Insurer's partner (assistance service) abroad, about which the Insurer has informed the Insured.

The Partner of the Insurer advises the Insured on his further actions, performs work in agreement with the Insurer

1 3 . OTHER RULES

13.1. For non-fulfillment or improper fulfillment of the obligations assumed, the parties are liable in accordance with the current legislation of the Republic of Uzbekistan.

13.2. The parties are released from liability if their failure to fulfill their obligations was caused by force majeure circumstances (such as natural disasters, fires, floods, earthquakes, etc., strikes, civil wars, etc.). The party affected by force majeure must

immediately notify the other party of the occurrence and possible duration of the specified circumstances within 30 (thirty) days.

13.3. If force majeure circumstances continue for more than 3 (three) months, each of the parties will have the right to refuse to fulfill obligations under the insurance contract.

1 4 . OTHER CONDITIONS

14.1. By entering into an insurance contract, the Policyholder certifies that it allows the Insurer, as a system manager, recipient of personal data and operator of personal data, to process the data of the Insured, and also guarantees that he has the consent of the Insured to the processing of these data by the Insurer, including sensitive personal data and identification (classification) codes of a person, for insurance risk quotation.

14.2. The obligation of the parties is not to disclose confidential information received in connection with the insurance contract, both about the second party and third parties, and also not to use it against the interests of the second party.

14.3. The text of these rules in Russian prevails over translations of this document.

14.4. All disputes that arise between the parties on the basis of the insurance contract (including on the basis of the terms of the policy and insurance rules) are resolved through negotiations. If a mutual agreement is not reached within 2 months, any dispute, misunderstanding or claim arising from the insurance contract relating to the contract or its violations, its termination or invalidity, will be finally considered in court using the legislative acts of the Republic of Uzbekistan.

14.5. Unless the parties have agreed otherwise in writing when concluding the insurance contract, or if within 2 (two) business days, counting from the date of conclusion of the insurance contract, the Insurer has not received written objections from the Insured, then the Insured shall also be recognized as a party to the insurance contract, in favor of which the insurance contract is concluded by the Policyholder, as a result of which the same terms and conditions of the insurance contract apply to the Insured, and the Insured is not recognized as a third party.

14.6. The parties to the insurance contract do not have the right to quote to third parties anything arising from the insurance contract, incl. possible and already existing, claims.

14.7. The contract is made in 2 copies, kept by each of the parties and having the same legal force.

14. 8. In everything that is not provided for by the terms of this Agreement, the parties are guided by the current legislation of the Republic of Uzbekistan , the reinsurance agreement and the rules of accident and illness insurance.

14.9. This agreement is governed by the Laws of the Republic of Uzbekistan . All disputes arising under this agreement in case of failure to reach an agreement through negotiations, will be resolved by the economic court of the Republic of Uzbekistan
